Where Hospice Care is Provided

Depending on your needs, hospice care is available in the following ways...

Home Hospice Care

Most patients receive skilled hospice care in their home or the home of a relative. This allows you to remain in familiar surroundings with family and friends nearby. Hospice Team members visit as needed. Your needs determine the frequency of visits. The Hospice Team also is available on-call for advice and support, 24-hours-a-day, seven-days-a-week.


Long-Term Care Facilities, Nursing Homes and Assisted-Living Communities

St. Luke's Hospice has contracts with several facilities in the area. The hospice team adds to the specialized care received at these facilities, including pain and symptom management.

Patients in need of acute care services or support may be admitted to St. Luke's Hospice House.

A team of experienced, compassionate caregivers staffs the Hospice House. They include a physician and nurses trained in hospice and palliative care, hospice social workers, a chaplain and a team of volunteers.

St. Luke's Hospice House was designed and built to meet the needs of our patients, as well as their families. It has 14 private rooms each offering a private bathroom, sitting areas for visitors and private patios. This facility is located in a quiet, park-like setting.

Beautiful handmade quilts are in every room, while plants and artwork contribute to the special atmosphere. The House also includes two large family rooms, a meditation room, counseling room and large gardens. A full kitchen offers families the ability to prepare favorite meals for their loved ones. Family members and loved ones are welcome to visit any time of the day or night.


Getting started with hospice care

A hospice program representative will contact you as soon as you, a family member or a physician makes a formal request/referral for services. Services can begin as soon as the referral is made or on any schedule that meets your needs.

Making a referral to hospice

Referrals to hospice can be made by physicians, family members or by you:

  • For patient referrals in Lehigh, Northampton, Upper Bucks and portions of Berks and Montgomery counties or for information about inpatient hospice care, please call 610-954-1100 or 800-211-4788 (toll-free).
  • For patient referrals in Carbon, Monroe and Schuylkill counties, please call 570-645-1840 or 866-349-1809 (toll-free).
