  • Please take your
    insurance card.
  • Semen analysis and sweat
    tests require appointments
    and are available only at
    the Bethlehem campus.

    weekday appointment.
  • Prescriptions written more
    than one year ago will not
    be accepted.
  • In bad weather, the hospital
    locations will be open. For
