Easy Steps to Get Your Medical Records

Your medical records are delivered to your doctor through a secured web site accessed only by medical personnel so that you never have to worry about picking up your own radiology tests or “films.” Your doctor gets high-quality digital images on a computer screen in the privacy of the office. And, with the help of fellowship-trained radiologists, your doctor can tell you what the results mean for your health. St. Luke's makes it easy for your doctor to determine the best course of treatment for you.

There may be times when your doctor will request you bring your records and images to your next appointment.

To make it easy to request your records, there are three ways you can contact St. Luke's Medical Records:

  • Attached is a Medical Information Release form that you can print, complete and mail to St. Luke's Medical Records, 801 Ostrum Street, Bethlehem PA 18015 or fax to 610-954-4784.
  • You can pick up this form at any patient care unit at St. Luke's Hospital - Bethlehem Campus and Allentown Campus. Then just fax it to 610-954-4784.
  • Call to request your records Monday through Friday between 8 am and 7 pm at 610-954-4719.

We will send your records back to you or the person designated on your record request form. St. Luke's charges only the State regulated charge for copies of the medical record.

To provide the best service we can, please allow some processing time for the following records:

X-rays on CDs – 48 hours

Medical Records – 2 to 5 days

Mammogram images to be mailed – 1 to 2 weeks
