Easy Steps to Get Your Medical Records

Your medical records are delivered to your doctor through a secured web site accessed only by medical personnel so that you never have to worry about picking up your own medical records, radiology tests or “films.” Your doctor gets high-quality digital images on a computer screen in the privacy of the office. And, with the help of fellowship-trained radiologists, your doctor can tell you what the results mean for your health. St. Luke's makes it easy for your doctor to determine the best course of treatment for you.

There may be times when your doctor will request you bring your records and images to your next appointment.

There are three easy ways to request your records from St. Luke's Medical Records:

We will send your records back to you or the person designated on your Medical Information Release form. St. Luke's charges only the State regulated charge for copies of the medical record.

A Medical Information Release form is available online or through each facility and needs to be completed. For your convenience we will forward your records free of charge to the necessary physician or facility for your continuation of care. St. Luke's abides by the State regulated fees for requests that are not for continuation of care. These requests are processed through an outside copy service.

Processing Guidelines

The following processing guidelines are for retrieval and reproduction of medical records for request completion.

  • X-rays on CDs – 48 hours
  • Medical Records – 2 to 5 days (processing time depends on information requested)
  • Mammogram images to be mailed – 1 to 2 weeks

The Medical Records Department will work with you and your physician to meet your needs.
